To Patients of Dr Crockett:

It has been my pleasure to serve our community for the past 39 years as a vascular surgeon. I have had the privilege of meeting a great many people and developing lifelong friendships with many patients. But now it is time for me to retire and pass along my patients to the other competent surgeons at Midland Surgical Associates.

Please know that all of your medical records are safely on file here at Midland Surgical Associates and remain confidential as per federal law. If you decide to take on a new surgeon outside of our practice, please call practice at 432-697-1061 to request your records transferred.

Over the past five decades I have watched many families in the area grow and have shared heartache and joy with hundreds of patients. I appreciate your loyalty throughout the years and will take the memories of all that I have experienced with me to my retirement. But medicine is a young man’s game and I need to take my final ride into the sunset.

My retirement is effective on September 1, 2020. I wish you health and happiness.
